Virtual Forum: Identifying Populations For Care Coordination Interventions: The Health Plan Perspective

Location: Online Via

Description: The goal of health care coordination is to meet individuals’ needs and preferences in the delivery of high-quality, high-value health care. For individuals with comorbid behavioral health disorders, effective care coordination begins with the health care systems’ ability to proactively identify at-risk individuals that may benefit from more intensive coordination efforts. During this virtual forum Samantha Sweeney, PharmD, MBA, Senior Managed Market Liaison at Otsuka Pharmaceutical Development & Commercialization Inc., will introduce the audience to the concepts of care coordination; followed by a case study presented by William Lopez, MD, CPE, Senior Medical Director, and Lisa Kay, MBA, MSW, Clinical Program Manager on the Clinical Program Development Team focusing on behavioral health integration in medical settings, both from Cigna Healthcare’s Behavioral Health Division. They will share Cigna’s effective approaches and strategies for identifying and implementing interventions in care coordination to increase effective treatment, adherence, and participation of individuals with comorbid behavioral health disorders.
