Increased physician employment by hospitals between 2012 and 2015 raised Medicare costs by $3.1 billion because a greater share of services that could be provided in an office visit were provided in a hospital outpatient department (HOPD). The $3.1 billion increase represented about 4.3% of the total increase in Medicare spending from 2012 to 2015….
