Description: This program is intended for practitioners who are currently practicing in or are interested in learning more about integrated care. Please join Connie Bartlett, DO and Clayton Chau, MD, PhD for this exciting program focused on: understanding the importance of integrated medical and behavioral health care; recognizing the importance of screening for behavioral health conditions in the medical care setting; reviewing specific integrated and collaborative care models; and exploring the future of integrated care.

Connie Bartlett, DO, is the Vice President of St. Jude and St. Joseph Heritage Medical Group as well as a practicing pediatrician. She has garnered numerous accolades including the prestigious OCMA Physician Excellence Award for the last 5 years. In addition, Dr. Bartlett has been consistently recognized for excellence in quality in service including clinical quality and patient satisfaction, as well as awarded top scores among her peers for clinical effectiveness in practicing evidence-based medicine.

Dr. Bartlett was elected chairman for the St. Joseph division in 2013. The position oversees strategic initiatives and coordinates finance and quality metrics for over one hundred multi-specialties prescribers from six different sites.  She is widely recognized as the expert in managing the challenges of integrated system within the scope of the current health-care reform mandates. She has also demonstrated leadership in process of care improvements, leading both horizontally across systems and vertically within her areas of responsibilities. Her faculty appointments include Associate Clinical Professor at Western University of Health and Sciences in Pomona, California.

Clayton Chau, MD, PhD

Clayton Chau, MD, PhD, is currently the Regional Executive Medical Director for the Institute of Mental Health and Wellness, St. Joseph Hoag Health System where he joined in January 2017.  Previously, he was the Senior Medical Director for Health Services at LA Care Health Plan, the nation’s largest public health plan.

Dr. Chau obtained his MD degree from the University of Minnesota and PhD in Clinical Psychology from Chelsea University.  He completed his psychiatry residency at UCLA/San Fernando Valley followed by a fellowship with the National Institute of Mental Health in psychoneuroimmunology focusing on substance abuse and HIV.  Dr. Chau has conducted international trainings in the areas of health care integration, health care system reform, cultural competency, veteran’s health, trauma, homelessness, and mental health policy.
