Description: “Treat the patient, not the disease” is a maxim of medical practice. But sometimes a laser focus on a particular disease or on a set of disease-related behaviors proves the best way to treat the entire patient and deliver appropriate, coordinated care. A population health approach, in which individuals are flagged for interventions based on criteria such as depression, substance use, or ED utilization, allows health systems to improve the quality of care offered to complex consumers while reducing the cost of that care.
So, what are health plans doing now to better manage their complex consumers’ behavioral health care needs—cost-effectively? During this webinar, Paul M. Duck and Deb Adler, both Senior Associates at OPEN MINDS, will share key drivers in population health management and discuss how health plans are getting complex consumers into care—that is, into appropriate, timely care—and bringing costs down.
