Live Program: Addressing The Transition Of Care For Patients With Schizophrenia: A Case Study Approach

Description: The objectives of this presentation are to review current challenges faced by patients with schizophrenia during the transition from hospital discharge to the community care setting, highlight published data on outcomes related to patient relapse, outpatient visit attendance, rehospitalization, and the value of clinical bridging strategies, review interventions to assist in discharge planning, and discuss antipsychotic medications and adherence issues.
