Description: Social determinants of health (SDoH) – like socioeconomic status, education, neighborhood and physical environment, employment, social support networks, and access to health care – can have drastic impacts on individual health, wellness, and quality of life.1 There are many stakeholder organizations attempting to address these social determinants – including federal and state governments, educators, law enforcement, and, of course, health care providers and payers.

In Part One of the SDoH Series, dig into the national SDoH domains, and hear three case studies on health care payer / provider initiatives aimed at tackling these factors, featuring:

Meredith Clemmons, LCSW, CCM

Meredith Clemmons LCSW, CCM, is the Associate Director of Product Development with Optum/United Health Group. Meredith has been with Optum/United Health Group for more than six years, first as a Senior Field Care Associate and then as a Manager of Medical and Clinical Operations. She has twenty years of experience in hospital, social service and managed care environments, with expertise in program and clinical leadership, behavioral – medical integration, behavioral health clinical operations and government sponsored health plans.

Ms. Clemmons received her Master’s Degree in Social Work from the University of Southern Mississippi.

Pamela Schwartz, MPH

Pamela Schwartz, MPH, is the Senior Director of Community Health Impact and Learning at Kaiser Permanente. Pamela has been with Kaiser Permanente for the last eighteen years, first as Senior Program Manager and then as Director of Program Evaluation. She has over twenty five years of experience working with health care delivery organizations, foundations, nonprofits, multi-sector collaboratives, schools and government agencies to transform health and health care. Pamela has numerous published works found in various journals such as, but not limited to, American Journal of Preventative Medicine, American Journal of Evaluation and American Journal of Health Promotion.

Ms. Schwartz received her Master’s Degree in Public Health with specialization in community health at San Jose State University.
